We support hospital discharges, admission avoidance and long term self-management, enabling patients to live better lives in their home environment. We also provide planned (Domiciliary) therapy to ensure that patients are assessed and supported early in the journey to maintain their optimal level of independence and hence avoid longer term concerns. We provide active rehabilitation to patients in their place of residence to maximise their functional potential and support them to remain independent within their homes.
We are seeking an enthusiastic and motivated Physiotherapist to join our busy and dynamic team in the South Glos Locality. We are looking for an Physiotherapist with experience of community working, to promote independence and choice for our service users and their carers. The post is based at Patchway Clinic and will be working within the Stokes Integrated Network Team and across the North of South Glos.
In return, we will give you the opportunity to develop specialist community skills and enhance your multidisciplinary working; we will provide you with regular training and supervision opportunities as well as the opportunity to develop your leadership skills in supervising Rehab Support Workers and Band 5 therapists.
You will be working within a friendly and supportive multidisciplinary team alongside other Occupational Therapists, Physiotherapists,Speech and Language Therapists, Rehabilitation Support Workers and Physiotherapy Technicians. We collaborate closely with our Community Nursing colleagues and other Acute, Community and Social Care partners. We offer a flexible service which covers extended hours operating 7 days a week, to support our service user needs.
Working arrangements include weekends and bank holidays on a rota basis.
